COAST ACADEMY OF JIU-JITSU
PARTICIPATION AGREEMENT, ASSUMPTION OF RISK, RELEASE OF LIABILITY & MEDICAL AUTHORIZATION

1. PARTICIPATION IN JIU-JITSU AND MARTIAL ARTS

I understand that Coast Academy of Jiu-Jitsu provides instruction and training in Brazilian Jiu-Jitsu, grappling, martial arts, practical self-defense, physical conditioning, mobility, drilling, partner exercises, takedowns, positional training, sparring, games, and related activities.

I understand that these activities involve physical contact and physical exertion and carry inherent risks of injury.

I voluntarily choose to participate in these activities.

2. ACKNOWLEDGMENT OF INHERENT RISKS

I understand that participation in Brazilian Jiu-Jitsu and related activities involves risks that cannot be completely eliminated without fundamentally changing the nature of the activity.

These risks may include, but are not limited to:

Falls and accidental falls
Being thrown, taken down, swept, pinned, or submitted
Physical contact with another participant
Accidental strikes or impacts
Joint, ligament, tendon, or muscle injuries
Sprains and strains
Dislocations
Fractures
Bruising and abrasions
Mat burns
Cuts and other skin injuries
Neck and back injuries
Head injuries and concussion
Loss of consciousness
Nerve injuries
Paralysis
Infection or transmission of communicable or contagious conditions
Allergic reactions
Exacerbation of an existing physical condition
Serious bodily injury
Permanent disability
Death

I understand that these risks may occur even when instructors, staff, and participants follow appropriate safety practices.

I knowingly and voluntarily assume the risks associated with participation.

3. TRAINING PARTNER CONTACT AND CONDUCT

I understand that Brazilian Jiu-Jitsu requires physical interaction with training partners.

I agree to follow instructor directions, train within my abilities, apply techniques with appropriate control, respect my training partners, tap or otherwise clearly communicate submission when appropriate, immediately release a technique when a training partner submits, immediately notify an instructor of an injury or safety concern, and refrain from intentionally harmful or reckless conduct.

I understand that training partners may have different levels of experience, size, strength, age, and physical ability.

4. ASSUMPTION OF RISK

I voluntarily assume responsibility for the risks associated with my participation in Coast Academy activities, including risks inherent to Brazilian Jiu-Jitsu and martial arts.

I understand that no training environment can eliminate every possibility of injury.

I am participating voluntarily and accept the inherent risks associated with the activity.

5. RELEASE OF LIABILITY

To the fullest extent permitted by applicable law, I release and agree not to bring claims against Gentle Art Enterprises LLC, doing business as Coast Academy of Jiu-Jitsu, and its owners, instructors, employees, independent contractors, agents, volunteers, representatives, successors, and assigns for claims arising from my participation in academy activities, including claims for personal injury, illness, death, or property damage, including claims based upon ordinary negligence, to the extent such claims may lawfully be released.

This release does not purport to release claims that cannot lawfully be released under California law.

I understand that this section is intended to be interpreted to the fullest extent permitted by law.

6. INSTRUCTOR AND ACADEMY RESPONSIBILITIES

I understand that Coast Academy and its instructors will make reasonable efforts to provide instruction and maintain a reasonably safe training environment.

I understand that martial arts instruction cannot eliminate every risk inherent in the activity.

I agree that instructors may modify, restrict, suspend, or terminate my participation when they reasonably believe doing so is necessary for safety.

7. HEALTH AND FITNESS

I represent that I am physically capable of participating in the activities in which I enroll.

I understand that Coast Academy does not provide medical diagnosis, treatment, or medical advice.

I agree to inform an instructor of any injury, illness, limitation, or condition that could reasonably affect my ability to participate safely.

I understand that I am responsible for determining whether participation is appropriate for me and for seeking medical advice when appropriate.

8. ILLNESS, CONTAGIOUS CONDITIONS & SKIN INFECTIONS

I agree not to participate when I have a condition that could reasonably place other participants at risk of infection or illness.

This includes, but is not limited to, contagious illnesses and suspected or known contagious skin conditions.

I agree to notify Coast Academy when appropriate and follow the academy's policies regarding return to training.

I understand that close-contact martial arts creates an increased possibility of exposure to communicable illnesses and skin infections despite reasonable precautions.

9. EMERGENCY MEDICAL AUTHORIZATION

If I become injured or experience a medical emergency while participating in academy activities, I authorize Coast Academy personnel to seek reasonable emergency medical assistance on my behalf when I am unable to do so myself.

This may include contacting emergency medical services and requesting transportation to an appropriate medical facility.

I understand that Coast Academy is not responsible for medical expenses incurred as a result of such treatment.

10. MEDICAL INSURANCE

I understand that I am responsible for maintaining my own health and medical insurance.

I understand that Coast Academy does not provide medical insurance to participants.

11. PERSONAL PROPERTY

I understand that I am responsible for my personal belongings.

Coast Academy is not responsible for lost, stolen, or damaged personal property except to the extent otherwise required by law.

12. PHOTOGRAPHY, VIDEO & LIKENESS

Coast Academy may photograph or record classes, academy activities, events, or promotional materials.

I understand that photographs or video may be used for legitimate academy purposes, including website, social media, advertising, promotional materials, and educational content.

13. MINOR PARTICIPANTS

If the participant is under 18, I represent that I am the participant's parent or legal guardian and have authority to enter into this agreement.

I have reviewed this agreement and understand the nature of the activities and risks described above.

I voluntarily authorize the minor participant to participate in Coast Academy activities.

I agree to the terms of this agreement on my own behalf and, to the extent permitted by law, on behalf of the minor participant.

I understand that a minor should not sign this agreement in place of the required parent/legal guardian consent.

14. ACADEMY RULES

I agree to comply with Coast Academy's reasonable rules and safety policies, including policies concerning appropriate training attire, personal hygiene, clean training equipment, clean hands, feet, and nails, jewelry and prohibited items, safe conduct, appropriate treatment of training partners, instructor directions, illness and contagious conditions, facility use, and children and youth supervision requirements.

I understand that violation of academy safety policies may result in suspension or termination of participation.

15. ELECTRONIC SIGNATURE

I understand that my electronic signature has the same intended effect as a handwritten signature to the extent permitted by applicable law.

I confirm that I have read this agreement, understand its contents, and have had the opportunity to ask questions before signing.

I understand that I am giving up certain legal rights by signing this agreement.

OPTIONAL MEDIA CONSENT

I authorize Coast Academy of Jiu-Jitsu to use photographs and/or video of me for academy promotional purposes. I understand that declining this consent does not affect my ability to participate in academy programs.
